Personal Care Aide Services

A personal care aide handles hands-on assistance with activities of daily living: bathing, dressing, grooming, toileting, transferring in and out of bed, and eating. Aides also cover light housekeeping, laundry, meal preparation, and grocery shopping when those tasks are tied to the care plan. Hours are authorized weekly and can range from a few visits to daily shifts.

Home Health Aide Services

Home health aides do everything a personal care aide does and add health-related tasks under nurse supervision, such as medication reminders, help with prescribed exercises, and monitoring vital signs. Skilled Nursing Visits

A registered nurse can come to the home for wound care, injections, catheter and ostomy management, and medication setup. Nursing visits are usually shorter and less frequent than aide shifts, and they are often paired with aide hours rather than replacing them.

Consumer Directed Personal Assistance (CDPAP)

CDPAP lets the person receiving care hire, train, and schedule their own caregiver, including an adult child, sibling, cousin, or friend. Since April 2025, every CDPAP consumer and personal assistant in New York registers and gets paid through a single statewide fiscal intermediary, Public Partnerships LLC. Spouses cannot be paid as personal assistants, and parents cannot be paid for a consumer under 21. Who Qualifies for Medicaid Home Care in Nassau County

Two separate tests apply. You have to clear both.

The 2026 Financial Limits

Anyone 65 or older, blind, or disabled applies under Non-MAGI rules, which look at monthly income and countable resources. Nassau County residents apply through the county Department of Social Services rather than the state marketplace.
